Apollo Root Cause Analysis, developed by Dean Gano, is a simple, yet powerful methodology for effective problem resolution. At the heart of this methodology is a cause and effect charting tool called a RealityChart. Also used is a companion software tool called RealityCharting® that significantly enhances the process and generates robust reports.

This major oil spill due to a leaking oil line in Prudhoe Bay, Alaska in 2006 is an example of an event based problem. (A combination of events culminated in the oil spill.) In addition to the environmental damage, this was very costly and caused negative publicity.
Our natural tendency is to place blame and to offer "pet" solutions. This approach is rarely effective in the long run.
Unfortunately, our news media sometimes reinforces the need to place blame, further pressuring organizations to deploy solutions before the problem is fully understood.
Use of Apollo Root Cause Analysis helps organizations to see deeper into the problem, and to expose a level of complexity beyond what they initially perceive.
