We frequently train Lean Six Sigma, ISO 9001 and similar specialists who want to continually improve their skills. This is because Apollo RCA represents the "state of the art" RCA methodology.

This major oil spill due to a leaking oil line in Prudhoe Bay, Alaska in 2006 is an example of an event based problem. (A combination of events culminated in the oil spill.) In addition to the environmental damage, this was very costly and caused negative publicity.
Our natural tendency is to place blame and to offer "pet" solutions. This approach is rarely effective in the long run.
Unfortunately, our news media sometimes reinforces the need to place blame, further pressuring organizations to deploy solutions before the problem is fully understood.
